Appointments:

We strongly encourage you to make an appointment, however, we also accept walk-ins.  If you’d like to schedule an appointment or find out how long the wait might be for a walk-in appointment, please call us at 541-481-7212.

If you are unable to keep your appointment, please call us as soon as possible to re-schedule.

Preparing for Your Appointment:

If you are a new patient, or if your medications have changed since your last visit, please bring your current medications with you to review with your provider.

Please also bring your insurance card and photo identification.

If you are a new patient, you can save time by completing your new patient paperwork before your appointment.  At your first appointment, there are three forms we will ask you to complete:

New Patient Information – English
Patient Demographics – English

You may also choose to complete an application for our sliding scale discount program in English or Spanish.

Fees / Insurance:

We accept many health insurance plans.  If you want to find out if we are an in-network provider for your health plan, please give us a call at 541-481-7212.

You do not need insurance to be seen at our clinic.

Columbia River Health offers a sliding scale discount program for individuals who meet eligibility.  This is determined based on income and number of individuals in your household.  If you’re uninsured, under-insured, or covering the cost of services will be a barrier for you, please call us to apply for the sliding fee discount program.

We also have staff on site who have been trained to assist people with signing up for health insurance, including Oregon Health Plan.
